New Client Information and Policies
Client Information Requirements: In order to facilitate a seamless intake process and establish client profiles within our system, we kindly ask that all new clients provide a valid cellphone number and email address at a minimum.
Health Intake: Prior to your scheduled session, we request completion of the health intake form. Failure to do so may significantly impact the time allocated for your session. This information is essential for session preparation and to ensure compatibility between therapist and client. Expectant mothers are kindly requested to include their current stage of pregnancy in the intake form.
Session Overview: Our standard 90-minute session includes a comprehensive assessment, including a postural evaluation if deemed necessary, alongside a detailed health intake. While the initial session may involve less hands-on treatment, it will be tailored to your individual health journey, offering ample time for consultation and information sharing.
Sickness Policy: If you suspect illness or are recovering from an illness, we kindly request that you reschedule your appointment. Late cancel fees will be waived under such circumstances, provided notification is given in advance. We extend the same courtesy and encourage open communication regarding any uncertainties about attending the scheduled session.
Cancellation Policy: Cancellation within 24 hours of the scheduled appointment will incur a 50% charge of the service fee as listed on our website. Payment will be processed using the card on file, and clients are responsible for ensuring the accuracy of their payment information. In the event of a medical or family emergency necessitating last-minute cancellation, no fees will be applied.
For cancellations due to sudden illness within 2 hours of the appointment, the full session rate will be charged. Clients with appointments scheduled as the first session of the day are encouraged to notify us via call, text, or email the evening prior if feeling unwell. Your cooperation in promptly canceling appointments at the first indication of illness is greatly appreciated.
